In this ancient Ayurvedic therapy, cloth boluses — tightly packed with specific medicinal herbs, herbal powders, or specially cooked medicinal rice — are dipped repeatedly in warm medicated oil and applied with firm, rhythmic pressure to the body in a systematic pattern.
Unlike ordinary massage, Kizhi works through two simultaneous mechanisms: the heat and pressure of the bolus opens the skin's pores and dilates blood vessels at the treatment site, while the medicinal herbs inside the bolus transfer their active compounds transdermally directly into the affected tissue. The result is a depth of therapeutic penetration that no topical application or oral medicine can match.
Described in the classical Ayurvedic text Ashtanga Hridayam under Swedana (sudation therapies), Kizhi is prescribed for conditions involving Vata and Kapha aggravation in the musculoskeletal system — arthritis, spondylosis, sports injuries, muscle stiffness, and neurological conditions affecting movement. Your therapist assesses your dosha constitution, specific pain patterns, affected joints or muscles, and any contraindications. Based on this, they select the appropriate Kizhi variant (Elakizhi for Vata conditions, Podikizhi for Kapha, Njavara for depletion) and the correct medicated oil for your case.
Fresh medicinal herbs or powders are tightly packed into cloth pouches. For Elakizhi, the leaves are briefly sauteed in medicated oil to activate their volatile compounds before packing. For Podikizhi, the herbal powder blend is measured precisely. Each bolus is tied tightly to create firm, even pressure when applied.
Medicated oil appropriate to your condition is warmed in a vessel. You lie on the treatment table in the optimal position for accessing your affected joints. A preliminary oil application is made to the treatment area to protect the skin and enhance the thermal conductivity of the bolus applications.
The boluses are dipped in the warm medicated oil and applied to your joints and muscles in a firm, rhythmic, rotational pattern. Your therapist maintains a steady cadence — alternating between two boluses to maintain consistent temperature, dipping them repeatedly in the oil vessel throughout the session. The pressure, rhythm, and direction of strokes follow classical Ayurvedic technique for the specific condition being treated.
After the initial full-body or full-joint coverage, your therapist focuses on the primary affected areas — spending additional time on the most problematic joints or muscle groups. The temperature of the bolus is carefully maintained throughout. At this stage many clients experience a deep burning sensation at the affected joint followed by profound relief as the heat penetrates the synovial capsule.
After Kizhi, a brief cool-down period is important — do not shower for at least two hours as the medicated oil and herbal residue continue working. Your therapist provides personalized post-treatment recommendations: dietary adjustments to avoid aggravating your dosha imbalance, specific stretches or yoga postures to maintain the mobility gained, and the recommended frequency for your treatment course.
